Fellowship Health Clinic medical patients are given the opportunity to meet with nurses for case management services at no cost to the patient.
Case management services include:
The case management team includes the Registered Nurse Case Manager and volunteer licensed nurse educators from the community.
At this time, Fellowship Health Clinic case management services are only available to patients treated by our medical clinic's physicians.
Case management services include:
- Chronic conditions support for patients with the chronic diseases of diabetes and high blood pressure.
- Integrated behavioral health services to improve coping skills, set and monitor goals, and encourage self-management of health issues.
- Coordination of care with medical providers, social services and support systems.
