Fellowship Health Clinic medical patients are given the opportunity to meet with social workers at no cost to the patient.
Social work case management services include:
Chronic conditions support for patients with the chronic diseases of diabetes and high blood pressure.
Integrated behavioral health services to improve coping skills, set and monitor goals, and encourage self-management of health issues.
Coordination of care with medical providers, social services and support systems.
The social work team includes the clinic's Licensed Master Social Worker, two master level interns from the University of Southern Mississippi's School of Social Work, and volunteer licensed social workers from the community.
Educational programs of the Fellowship Health Clinic social work department include Lunch and Learn, Group Lifestyle Balance, and Fit for Life. At this time, Fellowship Health Clinic social work services are only available to patients treated by our medical clinic's physicians.
